Have 'cleaned' the Amazon cave, but old Thrum won't acknowledge the fact. (all other quests relating to this have come up as complete).

Anyone know why this might be?

Don't worry about it. It pre-enchanted, and it only does +5 lock pick. It would be quickly replaced regaurdless.

After you kill the bat, go one or two steps north then one to the west. There will be a large hole on the wall. Walk into it or click on it.
